Pre-Surgery Information

Pre-Surgery Test/Procedure

After your provider schedules you for your test or surgical procedure, your insurance benefits will be verified. The registration staff at USMD will make a determination of your financial responsibility and contact you as soon as possible.

A member of our staff will then contact you to review patient history and/or schedule an appointment for pre-op testing. When called you will need to provide the following information:

  • Your primary care physician (name and phone number)
  • Your pharmacy (name and phone number)
  • List of regular medications
  • List of any drug allergies
  • List of any past surgery

Although individuals providing physician/ profes­sional services are members of our hospital staff, they are not employees of USMD Hospital at Fort Worth. Professional fees for their services, such as those provided by physicians, anesthe­siologists, radiologists and other specialists, will be billed to you separately from your hospital bill. These specialists and inde­pendent practitioners who provide professional services may not be participants in the provider network of your insur­ance or health benefit plan. If an out-of-network professional provides services, you will probably be responsible for those charges.

Contact your plan administrator if you have questions about the status of any hospital based provider’s participation in your plan network.

If you are unable to keep the appointment for Pre-admission Testing or surgery, please call (817) 433-9100 to cancel or make further arrangements. Please notify the surgeon of the cancellation as well.

Pre-Op Dietary Checklist

An anesthesiologist will make every effort to call the night before the procedure to discuss the patient’s medical history and preparation for anesthesia. For patient safety the following are NOT ALLOWED AFTER MIDNIGHT (unless otherwise indicated). Failure to comply may result in cancellation of your surgery.

If you have ANY questions regarding the before or after-care of surgery, please contact the surgeon.

MEDICATIONS* – Taken as prescribed with NO MORE THAN 2 TABLESPOONS OF WATER. ALLOWED until 2 hours before arrival.

*INHALERS FOR ASTHMA ALLOWED at all times.

CLEAR LIQUIDS (Water, Pedialyte, apple juice, Gatorade, clear Jell-o). ALLOWED until 2 hours before arrival.

SOLID LOW-FAT FOODS (Formula, milk, cereals, breads, fruit juice with pulp).  ALLOWED until 6 hours before arrival.

SOLID HIGH-FAT FOODS (Meats, cheeses, fried foods).  ALLOWED until 8 hours before arrival.

BREAST MILK – ALLOWED until 4 hours before arrival.

Day of Surgery

Please bring the following the day of surgery:

Documentation

  • Payment (if applicable)
  • Driver’s license
  • Insurance card
  • List of allergies and medications
  • Any special information from the surgeon (e.g., X-rays, consents, orders)
  • Copy of advance directives (if applicable)  (e.g., a living will or durable power of attorney)
  • Legal guardianship papers (if applicable). A parent/guardian must be available to sign all hospital required documents. If we can not verify legal guardianship the procedure will be rescheduled, pending appropriate documents being provided.

Personal Items

  • Do not wear make-up to hospital
  • Shower the morning of surgery (Do not shave the surgical site)
  • Leave all valuables and jewelry at home (including wedding bands and body piercing jewelry)
  • Current medications in original bottles (labeled with prescribed dosage)
  • Any personal assistive devices (e.g., CPAP, walker, oxygen, inhalers, etc.)
  • Contact lens/glasses cases (if applicable)
  • Change of clothes, extra diapers, or pull-ups
  • Comfort items (e.g., blanket, doll, etc.)

Post-Op

  • Wear loose comfortable clothes. If surgery will require a large dressing or brace/cast, please wear clothing that will be easy to get on. (The patient may wear his/her own pajamas to the facility, if desired).
  • An 18-year or older licensed driver available to drive the patient home. (This will be verified prior to the start of the procedure. If a legal aged driver is not available the procedure will be rescheduled.)
  • An overnight bag with toiletries.
